INSTRUCTIONS FOR ONLINE APPLICATION FORM FOR REGISTRATION NURSERY, L.K.G. & U.K.G. (FOR 2024-25)

| 1 | Application form for Registration of Nursery, L.K.G. & U.K.G. should be made online through the online Registration form available on the school website www.stmarysagra.org from Wednesday, 6th December 2023 |

| 2 | Please log on and follow the instructions given on our website to fill the online registration form. |
| 3 | Fill in all the required information as indicated in the form. Incomplete form will not be accepted. |
| 4 | Fill the application form very carefully, as no correction would be possible after submition of form. |
| 5 | Kindly Submit the hard copy of online Registration Form along with the following documents from Saturday 9th December 2023 during office hours (09:00 a.m. to 01:00 p.m.). |
| 6(a) | Passport size Photograph of the child along with parents. |
| 6(b) | Photostat copy of Birth Certificate with the name of the child issued by Nagar Nigam. (Form No. 5) |
| • | Complete Name of your child should be printed on the Nagar Nigam Certificate. |
| • | Date of birth of your ward will not be changed in any circumstances after the submission of form. |
| 6(c) | Address proof (Photocopy of any one of these : Aadhar Card / Voter ID Card / Passport / Electricity Bill / Driving License , of parents ). |
| 6(d) | For Christians – Photocopy of Baptism certificate. |
| (Original copies of all documents and photograph need to be brought at the time of interaction.) | |
